Online Admission Procedure

ONLINE REGISTRATION PROCEDURES

All newly admitted students into various academic programmes of the Lagos State University of Science and Technology are requested to please note the following approved procedure for their on-line clearance/registration as bonafide students in the University.

Procedure

  1. Student prints out JAMB Admission Letter
  2. Students proceed to pay Acceptance fee
  3. Students print out the following Forms in triplicate from the University portal for completion:
  4. Students proceed to his/her Department for clearance with the Academic Advisor

            Form

  • 001         Student’s Data Form;
  • 002        Registration Clearance Certificate Form;
  • 003        Matriculation Oath / Parent’s Indemnity Form;
  • 004        Undertaking Form;
  • 005        e-Certificate Processing Form; and
  • 006        Student’s Identity Card Form.
  1. Students proceeds to his/her Department for clearance with the Academic Advisor
  2. Student proceeds to his/her College Officer for screening
  3. Student submits all duly completed Registration Forms in triplicate and six (6) photocopies of credentials to the College Officer to enhance opening of Students’ Files.

Checklist of Documents for Submission in Triplicate

  1. Form 001 Student’s Data Form;
  2. “     002        Registration Clearance Certificate Form;
  3. “     003        Matriculation Oath / Parent’s Indemnity Form;
  4. “     004        Undertaking Form;
  5. “     005        e-Certificate Processing Form; and
  6. “     006        Student’s Identity Card Form.
  7. JAMB Admission Letter;
  8. Birth Certificate/Sworn Affidavit in respect of age;
  9. JAMB Application Form;
  10. Post-UTME Application Form;
  11. Post-UTME score slip;
  12. Entry qualifications (O’level Results);
  13. Evidence of Acceptance/School Fees payment; and
  14. Twelve (12) Passport size photographs;
